You can clearly see him hit some sort of control near the back of the unit both times it changes directions. Why that control exists is beyond me … other than to make this video, I guess.

Not really. When the machine goes forward and he slams his head and falls backwards, it changes direction at the same time he is falling backwards with his hands in the air. I agree this is probably staged, but I don’t see what you’re seeing.

At the beginning he taps the control on the left side of the machine to make it go forward and then shortly thereafter he taps a control on the floor to make it go backwards (he hits his head on the front when its transitioning from forwards to backwards) and then he taps the control on the left side of the machine at the end for the nut shot.

I don’t think I’ve ever seen a treadmill with controls there, but I’ve only used maybe 3 in my life.
