I’ve never played a rhythm game, but looking at this video I have to ask myself:
How does someone get this fast? Do these types of skills carry over from song to song? Obviously you can practice individual tracks over and over, but if we gave this guy a brand new song to play against, would he be able to fly through most of it that fast, or would he be completely unable to play through any of it it until he learns that one specific song?

Do people that play these songs this fast watch the arrows at all?

Yes, but they don’t consciously think about hitting the keys. Look at a really good hammer dulcimer player’s hands at some point.
