Suggestions Wanted - Electronic Forms

Hey guys,

I wanted to take a paper form that gets filled out and delivered to various managers and make a form using my existing WordPress site. I want to be able to have the form submit to an email address based on the department field chosen at the top, and have some light branching in terms of “if you check this box these required fields will appear” and to have the finished form optionally look nice/decent and be sent as a PDF for approval.

Is anyone using any plugins they would recommend for something like that, or some other options? I don’t mind spending a little cash, like $50-70, but would rather go with a free option if that’s on the table.

Google Forms would cover your need very easily, though it’s not directly integrated with WordPress. I’ll see if I can’t dig up the current forms plugin we’re using on our WP site at work, which gets the job done perfectly well.

edit: we use Formidable, which I do like, though it’s fairly powerful/complex. But tons and tons of options.

https://contactform7.com/ is free but wont do what you want out of the box. You can add the needed functionality with add-ons, eg https://wordpress.org/plugins/cf7-conditional-fields/ adds conditional fields.

I’ve done most of that with Gravity Forms, and I think it supports the PDF feature with an additional add-on.

Any secure info (personally identifiable information, etc.) on that form? Once you go webform (whichever solution you pick) that info will then accessible in new ways, which could create problems down the road for your security folks/person. Just something to keep in mind.
