I went down a rabbit hole after reading that it was supposed to be the cover and it looks like it may have been intended to be used down the road on a re-issue or something? 1983 catalog has Haunted House with the re-issue / re-print box style and the art above: http://atariage.com/forums/topic/236356-monaco-new-zealand-catalogue/

Wow, that’s crazy. Yep! I threw in the ESRB to help put a date on it but that wasn’t needed it turns out.
I have various horrible names coming to my mind, but I can’t help but have Disc World’s name floating in here since the first snap. Not sure why, as it’s hardly sci-fi !

Nope, a bit more obscure than both of those. Sold maybe a thousand or so copies. I was tempted not to even do this one but in a thread a decade or so back someone on Qt3 mentioned it.
I’m pretty sure I know this, as it seems pretty squarely in my wheelhouse in that period, but I can’t quite place it.
